Following are the steps.
1. Open an account in US$ in Pakistan (Let's say Meezan bank)
2. Open an account in Washington Mutual in USA )now part of Chase Bank) , they have free wire transfer option.
3. Get SWIFT code from Your bank in Pakistan and provide it to Washington Mutual and execute Wire transfer.
4. Money will be in your Pakistani bank within 48 hours.
5. There is no fee, neither at transmit nor on recieving end.
I have executed an experimental run with $200 and can see it within 48 hours online in my Pakistani bank.

There is a small fee on the other end though. If you send 200.00, you will receive a a few dollars less (10-20). Its one price for any amount of transaction, whether you send 200.00 or 200,000.00. Washington Mutual does not charge, but the bank on the other side does, atleast MCB and Askari bank does charge a small fee, but its a lot better option than others.

Its the same process from the other side. Its a wire transfer from Bank in pakistan to a bank in US. Just give them the Swift code, Routing number and Acct number. I am not sure why and how anyone can stop you from transferring your own legal money anywhere in the world, in any amount?

Atleast Bank Al Habib and Meezan bank do not charge any thing at the receiving end. If a middle bank is involved than they may charge a fee.
